With this Smile Pro Plan Program I agree
- This plan cannot be combined with any form of dental insurance.
- To purchase the Smile Pro Plan, members understand they must have a zero balance ontheir account.
- Members are responsible for taking advantage of the allowed services within the year ofthe plan. Any benefits left unused cannot be carried over past the 365 day mark. Members understand any benefits not utilized within the year of purchase are forfeitedon the date of plan expiration.
- Treatment must be paid in full on the date of service, otherwise the 10% discount will not be applied
- If a member chooses they no longer want the Smile Pro Plan they may cancel, however ifany treatment has been applied under the plan no refund will be given.
- This plan is one year in length, starting on the date purchased. All prophy appointmentsmust be scheduled on contract date to stay in accordance with routine care.
